Start with your model

Which care business are you building?

Each model has a different regulator, payer and scope boundary. Choose yours to see where the work starts.

Nonmedical home care

Companion, homemaker and safety-presence services kept inside a lawful nonclinical scope. Often the fastest controllable launch.

Usual payer
Private pay, long-term care insurance, some waiver programs
Main regulatory issue
State home care rules; scope must stay nonclinical
First question we answer
Is every service you plan to market truly nonclinical?

Typical starting point: Foundation Advisory

The authorization chain

An LLC is not a license. A license is not a paycheck.

Each link has to be active before you deliver or bill the related service. We plan the whole chain in order.

  1. 01

    Entity

LLC, EIN, bank account and operating agreement.

≠ Not a license to serve anyone

  2. 02

    License

State authorization for the exact service and setting.

    ≠ Not LLC formation

  3. 03

    Enrollment

Registration with Medicaid or Medicare under the right provider type.

≠ Not a contract with every plan

  4. 04

    Credentialing

Verification of ownership, staff, insurance and compliance.

    ≠ Not guaranteed network entry

  5. 05

    Contracting

A signed agreement with a payer, agency, facility or program.

    ≠ Not a referral

  6. 06

    Authorization

Approval for a named client, service, units and dates.

≠ Not permission to bill any amount

  7. 07

    Claims payment

Payment after a correct claim is adjudicated.

    ≠ Not guaranteed revenue

Where launches go wrong

Common problems, and what we do instead

ProblemA lease is signed before zoning and occupancy are cleared.

Sober living, shared living and residential programs can stall for months on property questions.

What we doClassify the model and clear the property first.

Your roadmap confirms license type, zoning and occupancy questions before you commit to a property.

ProblemAn LLC or NPI is treated as permission to operate.

Services delivered or billed before authorization create repayment and licensing risk.

What we doMap every link of the authorization chain.

You see which approval unlocks which service, and in what order to pursue them.

ProblemA staffing agency runs out of cash in its first quarter.

Workers are paid weekly while facilities pay in 30 to 60 days.

What we doSize payroll float and price from burdened cost.

Your budget and bill rates account for taxes, insurance, screening and overhead.

ProblemA generic policy binder fails a survey.

Policies that do not match the license, payer or forms cannot be shown in practice.

What we doTailor policies to your state and tie them to forms.

Each policy has an owner, a review date and a form that proves it is followed.

What you receive

Real deliverables, not a generic binder

Straight answers

Cost, scope, timelines and responsibilities

What does it cost to start a care business?

It depends on the model. A private pay companion agency can start lean; a staffing agency needs payroll float that often runs $50,000 to $250,000 or more because facilities pay in 30 to 60 days; a Medicare-certified home health agency needs capital to carry it through licensure, certification and survey. Foundation Advisory includes a startup budget and 12-month cash plan built from your own numbers.

What is included, and what is not?

Each package lists its deliverables, sessions and support period, and your written proposal confirms them before payment. Consulting fees exclude government and licensing fees, insurance, background checks, accreditation, property, vehicles, legal services, CPA services and other third-party costs unless the proposal expressly includes them.

How long will licensing take?

Regulators and payers set the timeline, and it varies by state, model and how complete the application is. We give you a sequence with dependencies so you know what can run in parallel. We do not promise approval dates.

What do you do, and what do I do?

We prepare the roadmap, budget, policies, forms and application documents, and we coordinate the work. You own the business: you sign and submit applications, hire staff, buy insurance and make final decisions. Your attorney and CPA handle legal opinions and tax filings.

Will Medicare pay for my nonmedical home care agency?

Generally no. Medicare does not pay for most long-term custodial care or ordinary nonmedical support. Nonmedical agencies usually start with private pay and build a separate Medicaid waiver pathway.
