Healthcare staffing agency startup support.
The strongest launch model is a W-2 employer structure with a narrow first role and geography, enough payroll capital, and signed facility agreements before aggressive recruiting.

Get the order right
- Foundation: entity, EIN, bank, accounting, employer registrations, workers’ compensation, insurance and staffing registrations.
- Market choice: one service area and two to four roles such as caregivers, direct support professionals, CNAs, LPNs or RNs.
- Client readiness: master services agreement, rate schedule, work orders, timekeeping and invoicing.
- Workforce readiness: no worker is placed with an incomplete or expired file.
- Controlled launch: high service levels and weekly reviews with your first five facilities.
Price from burdened cost, not competitors
Bill rates start from the fully burdened hourly cost: wage, payroll taxes, workers’ compensation, paid leave, recruiting, screening, training, technology, insurance, bad debt and overhead. In our illustrative example a $24.00 wage carries an estimated burdened cost of $34.54 per hour before margin.
Who pays a staffing agency
A staffing company normally invoices the facility, agency, group home, clinic or hospital receiving the labor. Medicare and Medicaid pay the enrolled provider for covered services, not a staffing company supplying workers. We never market staffing as guaranteed Medicare or Medicaid reimbursement.

Questions founders ask
How much working capital does a staffing agency need?
Payroll float is the main cash risk. Planning ranges in our package run from $50,000 to $250,000 or more depending on roles, volume and client payment terms.
Do you recommend 1099 contractors?
We recommend a W-2 employer model for healthcare staffing because worker-classification and joint-employment risk are high. Confirm your structure with employment counsel.
